Escorted Mobility Van

The Escorted Mobility Van compliments the existing Volunteer Escorted Transportation program and helps remove the barriers experienced in accessing medical care.

The van is capable of transporting wheelchairs, a variety of other mobility assisted devices, and additional passengers.

Please give our office as much information as possible, when you call.

You made my birthday! It was so nice having Mom come from the retirement home. It was such a nice surprise and so nice to have pictures taken of the WHOLE family while she was there!!! Thank goodness we have a Mobility Van now for personal appointments. April 2016

Escorted Transportation

Volunteer drivers are available for long distance and local trips for medical appointments, shopping or social trips. Escorted transportation includes kitchen table to kitchen table service where the client is picked up at their home to take them to their appointment, escort them inside, wait during the appointment and escort them home.

A client called and said "I don’t go out much especially during the winter months and it use to upset me if I thought I had a doctor's appointment to attend. After signing up with CCCH I no longer worry…….I call the office and it gets set up so quickly and easily and all the drivers are wonderful to help. I am content in knowing I will arrive back home safe and sound. Sept 2015

Fee: per trip. Please be aware there is a rate schedule that is calculated based on distance, number of stops, and gas prices.

To book a ride: Please contact the office at least 2 business days in advance of the trip, if possible.
Please be prepared to tell us the:

  • Date
  • Time
  • Destination address
  • Doctor's name (if applicable)
  • How long your visit may be
  • How many stops may be required
  • If you require assistance in and out of the vehicle .
  • This information assists us in locating an appropriate vehicle, preparing our volunteer drivers for the destination and the amount of time required for the trip.

    Having to attend Dialysis 3 x’s a week is stressful. My family takes me as much as they can but sometimes they can’t and knowing that I can call CCCH for s driver is simply piece of mind. June 2015
