FROZEN MEALS ON WHEELS ORDER FORM

Please complete the form below if you would like to order any of our frozen items from our frozen menu. Entrees are $6 each, soup and dessert are $2 each. Please allow 2 business days to process your order. A confirmation email will be sent to confirm the amount due. If you are a client we will send an invoice the following month. If you are not a client, payment is due when items are picked up. Please call if you have any questions!
